The College provides reporting tools for students, faculty and staff to submit an official report of a situation, concern or incident that took place on or off-campus involving Kishwaukee students. Please find the appropriate forms below.
Reporting Forms
This form is for faculty to report incidents of cheating, plagiarism or other academic misconduct identified in the Student Code of Conduct.
This form is only to be used if you have been identified as a Kishwaukee College Campus Security Authority (CSA). The form is required to be completed by CSA’s when a student or employee discloses a Clery Act Crime has been committed on campus or at one of our identified off-campus locations per our identified geography.

This incident report is to document all student and student-athlete injuries and accidents. An incident report should be completed at the time an incident occurs, no matter how minor the injury is.
